How DoesElite iQ Work?
What is PMOS?
Polyendocrine metabolic ovarian syndrome (PMOS) is the new name for polycystic ovary syndrome (PCOS), adopted in 2026 after an international consensus published in The Lancet. The old name focused on ovarian cysts, which many women don’t have. The new name describes what it really is: a hormonal and metabolic condition that affects the whole body, and about 1 in 10 women. Its exact cause is still not fully understood, but the common thread is insulin resistance, often while blood sugar stays normal. That places PMOS in the same family as prediabetes and type 2 diabetes.
Why does PMOS cause unwanted hair?
The answer often starts with insulin. Insulin resistance signals the liver to make less sex hormone-binding globulin (SHBG), the protein that binds testosterone and holds it in reserve. With less SHBG, more testosterone circulates free and active, so its effects rise even when total testosterone looks normal on a lab test. High insulin also prompts the ovaries to produce more androgens. Hair follicles respond by turning fine, light hair into coarse, dark hair on the chin, upper lip, jawline, neck, chest, abdomen and back. This is called hirsutism, and it is one of the most recognizable signs of PMOS.
Does laser hair removal work with PMOS?
Yes. Coarse, dark hair responds well to laser. Expect more sessions than average and occasional touch-ups, because insulin resistance keeps driving new growth until it is treated. We use the Cynosure Elite iQ with its 1064 nm Nd:YAG wavelength, which is safe for every skin type. More about laser hair removal at Serenity.
How many sessions will I need?
Often more than the usual 9 to 12 for the face, plus touch-ups a few times a year. We’ll give you a realistic plan at your consultation.
Why treat the insulin resistance too?
Because it is the root of the problem. As insulin sensitivity improves, SHBG recovers, free testosterone falls, and new hair growth slows, which helps laser results last. It protects your long-term health too, since PMOS raises the risk of type 2 diabetes. A normal blood sugar test doesn’t rule insulin resistance out.
How does Serenity help with the metabolic side?
Dr. O’Connell evaluates your weight, insulin resistance and metabolic health. Metformin has long been the standard medication for insulin resistance in PMOS. Newer GLP-1 medications such as semaglutide and tirzepatide also improve insulin resistance while helping with weight loss. He prescribes them when they’re right for you, as part of our medical weight loss program.
Do I need a PMOS diagnosis first?
No. Laser hair removal works either way. At your consultation, Dr. O’Connell explains how insulin resistance may be driving your hair growth and what to ask your primary care provider or gynecologist to check, so insurance covers the lab work. There’s no single test for PMOS, and routine panels often miss it, so he’ll point you to fasting insulin, free testosterone and the ratio of LH to FSH.
What happens at my first visit?
A complimentary consultation with Dr. O’Connell. He reviews your hair, skin, history and medications, builds your hair removal plan, and explains what may be driving your hair growth.
